The Transitional Living Community (TLC) Program’s mission is to assist individuals in obtaining and maintaining a healthy lifestyle which promotes individual growth and life enhancement, as well as the development of valued social roles. As individuals work through the stages of recovery the long term goal of abstinence can be obtained.
TLC is a fourteen-bed twenty-four hour staffed residential program for adults who are homeless, and have a co-occurring diagnosis of mental illness and substance abuse.
Services will be guided by value-based principles, which include:
· The belief that individuals have unique needs, goals, attitudes, and expectations
· The belief that messages of hope and empowerment foster recovery
· The belief recovery is possible
· The importance of cultural and gender competency and sensitivity is delivering services
· The importance of eliminating stigma and promoting recovery awareness within our communities
· The importance of member-driven goals in defining support and assistance
TLC services include: Vocational and educational supports, case management, person centered planning, and integration to community resources, daily living skill instruction and medication monitoring.
