The TRANSITIONAL APARTMENT PROGRAM (TAP) is a scattered site apartment program designed to provide community placement and support for individuals under the auspices of the Psychiatric Security Review Board (PSRB) and individuals on parole or probation. Staff works closely with State Forensic Monitors employed by Southeastern Mental Health Authority. TAP is wholly funded by the Connecticut Department of Mental Health and Addiction Services (DMHAS), and therefore only accepts individuals sponsored by DMHAS.

TAP is staffed by three Residential Case Managers, one Coordinator, and one Unit Director. Intensive services include, but are not limited to:

· Conditional Release monitoring and review
· Probation stipulation monitoring and reporting
· Advocacy for treatment
· Assessment of rehabilitation needs
· Assistance in developing person-centered service plans
· Goal development
· Observation of medication self-administration
· Weekly clinical group sessions for PSRB members
· Crisis intervention
· Teaching daily living skills
· Providing social supports and awareness
· Linking to community resource

As program participants successfully adjust to community living, supports are modified. On-site staff support is provided from 8:00 a.m. to 7:00 p.m. on weekdays and 8:00 a.m. to 12:00 p.m. on weekends and holidays. Twenty-four hours per day, TAP members can access the TAP beeper system for emergency assistance or needed supports.

As part of the program, TAP uses existing Reliance House programs to provide a wide range of supports and resources adapted individually to meet the needs of each program participant. Programs available include Career Services, Educational, and Teamworks Clubhouse.
