The OUTREACH PROGRAM provides services for the Norwich area and is for people who are:

· Homeless or at risk of homelessness who have experienced pro-longed mental illness
· Adults over the age of 18

Services include:

· Outreach and engagement to services to meet basic needs (shelter, food, and clothing)
· Assistance in obtaining entitlements (filing for Social Security or General Assistance, fuel assistance, etc.)
· Assistance in linkage to Reliance House support services (case management, social, educational, and vocational) and to community agencies (clinical services, food banks, furniture, etc)
· Landlord mediation, advocacy, and/or referral to appropriate legal and financial support services
· Two four-bed shelters for adult single males and females who suffer from severe and prolonged mental illness

· Nationally, 65% of these individuals have experienced mental illness
