Any individual seeking services at Reliance House (either by personal or family request, or
referral from another agency) must first complete an INTAKE. The goal of the Intake process is to interpret the needs of the individual seeking services to see if Reliance House can best serve that individual to its fullest capacity. The individual must fit the target population
determined by the
Department of Mental Health and Addiction Services.
The Intake Committee:
· Receives and responds to all initial requests for services (excluding Respite and Jail Diversion).
· Centralizes the response, assignment, and outcome of all intake re-quests that come into Reliance House.
· Records all necessary statistical data for internal documentation, as well as all intake data required by our funding source.
· Furnishes an annual report that traces the activity and trends of the program.
· Provides education and information to the community about Reliance House services.
