
The BRIDGE PROGRAM is a 24-hour residential case management program that supports people who have experienced mental illness who are living in the community. The program has staff on duty on all shifts, 7 days per week. The program is unique in that its staff office is based in a five-bedroom house in Jewett City where five people are able to live and receive intensive support from staff while sharing house facilities.
Other program members reside in apartments in the Jewett City area where staff meet them for skills training, supportive intervention and med-monitoring. The program also provides:
· Transportation
· Medication monitoring
· Recreational activities
· Evening groups
The program uses the individual approach by supporting members at their own level of need. The ultimate aim of the program is to help the person live as independently as possible in the community. When a person feels ready to leave the program, they are referred to a less intensive support service such as a general case management program. This allows the program to maximize its support services for the people who need it most, while concurrently helping people grow and achieve their goals.
