The Community Support Team is comprised of the former case management & residential support programs, Goal Development and Advocacy (GDA) and the Scattered Site Apartment Program (SSAP) and is comprised of two teams: CST I and CST II.

 

Community Support services are highly individualized and are based upon each person’s unique strengths and needs. The team assists in identifying and accessing community resources. They help to develop and practice skills necessary to maintain housing and to succeed in employment and education.  They work to develop coping strategies to manage stress and prevent crises. Service Coordinators provide the appropriate level of support necessary for members to remain in the community, assist members in developing goal plans, guide members in developing skills to enhance quality of life while living in the community and provide links to needed or requested services as determined on an individual basis. Other services include: crisis intervention, social support, budgeting assistance, transportation, advocacy, daily living skills instruction, medication monitoring, and integration to community resources.
